United Surgical recently billed my Insurance company (United American) $6369 for the facility where my cataract removal was performed. They paid $4900 which represent 80% of the remainder after my $500 deductible responsibility. The surgeon billed $3000 , which they paid 100%, plus $995 for the anesthesia. The total received by them was $10,584. This amount is in contrast with a $2700 amount they told me was their charge prior to scheduling surgery. The $6369 amount for their facility actually billed was made known to me to me 15 minutes before surgery and it was, sign a note for it or you don't get surgery.

The costs quoted for Cataract surgery are about twice current "average" cash price (the minority), and about quadruple current "Medicare" price (which is the majority).
